Join us for a free Advance Care Planning workshop designed to help you think about, talk about, and document your future care wishes. Planning ahead isn’t just for later in life—it matters at every age and stage.

During this session, you’ll learn what Advance Care Planning is, why it’s important, and how to get started. We’ll walk through how to reflect on your values and wishes, choose and prepare a Substitute Decision Maker, and have meaningful conversations with family, friends, and healthcare providers. You’ll also be introduced to practical tools and resources to help guide you through the process.

Advance Care Planning can bring peace of mind, reduce stress for loved ones, and ensure your voice is heard if you are ever unable to speak for yourself.

This session is open to everyone in the community. Whether you’re planning for yourself, supporting a loved one, or simply want to be informed, you’re welcome to attend.
